Phase 1 — RELEASE (Weeks 1–13)
Confronting the Weight of the Wound
Goal: Help mothers release guilt, shame, and unhealthy relational patterns; begin separating identity from the crisis.
Objectives (3):
Benchmark: By Week 13, ≥20% decrease in guilt/shame/stress (PSS or adapted shame indices) and the mother can name 2+ patterns she has begun to release.
Primary modalities: Psychoeducation, CBT, ACT, EMDR (targeted), Family Systems, trauma-informed mindfulness.
Core interventions: Trauma mapping; cognitive restructuring of self-blame; narrative reframe (“failed mother” → “healing mother”); EMDR for guilt cues; education on family roles in addiction; boundary scripts.
- Identify and process guilt/shame;
- Recognize enabling/codependency and begin letting go;
- Establish foundational boundaries and self-compassion.

Phase 2 — RECOVER (Weeks 14–26)
Rebuilding the Self
Goal: Build emotional stability, regulation, and resilience amid ongoing uncertainty.
Objectives (3):
Benchmark: By Week 26, ≥25% reduction on GAD-7/BDI-II and consistent use of 3+ coping/skills in daily life.
Primary modalities: DBT (distress tolerance, emotion regulation), CBT (catastrophic thinking), ACT, Mindfulness-Based Interventions.
Core interventions: Resilience plan; crisis scripts; mindfulness and body scans; thought records; boundary enforcement drills; relapse-response playbook (for the child’s setbacks without maternal collapse).
Objectives (3):
- Learn/apply DBT/CBT/ACT regulation skills;
- Establish sustainable self-care (sleep, nutrition, rest);
- Implement resilience strategies for anxiety and burnout.
Benchmark: By Week 26, ≥25% reduction on GAD-7/BDI-II and consistent use of 3+ coping/skills in daily life.
Primary modalities: DBT (distress tolerance, emotion regulation), CBT (catastrophic thinking), ACT, Mindfulness-Based Interventions.
Core interventions: Resilience plan; crisis scripts; mindfulness and body scans; thought records; boundary enforcement drills; relapse-response playbook (for the child’s setbacks without maternal collapse).
Phase 3 — RENEW (Weeks 27–39)
Restoring Connection & Attachment
Goal: Reclaim identity, self-worth, and life purpose beyond the caregiving crisis.
Objectives (3):
Benchmark: By Week 39, ≥30% improvement on RSES (self-esteem) / life satisfaction measures and articulation of 1+ concrete personal goal independent of the child’s condition.
Primary modalities: Narrative Therapy, ACT (values), EMDR (identity injuries), Self-Compassion therapy, expressive therapies.
Core interventions: Values audit; “who am I now” identity work; future visioning; role-plays for healthy bonds; creative expression; forgiveness processes (self/system/child when appropriate).
Objectives (3):
- Reconnect with values/identity;
- Reduce self-blame; cultivate self-compassion;
- Set new personal goals and relational intentions.
Benchmark: By Week 39, ≥30% improvement on RSES (self-esteem) / life satisfaction measures and articulation of 1+ concrete personal goal independent of the child’s condition.
Primary modalities: Narrative Therapy, ACT (values), EMDR (identity injuries), Self-Compassion therapy, expressive therapies.

Phase 4 — RISE (Weeks 40–52)
Empowerment, Advocacy & Legacy
Goal: Sustain healing, strengthen family systems, and embody empowered motherhood.
Objectives (3):
Benchmark: By Week 52, ≥40% improvement in resilience/stress regulation (PSS, PCL-5) and demonstration of 1+ sustained lifestyle/relational change.
Primary modalities: Family Systems therapy, CBT relapse-prevention, strengths-based coaching, group therapy/peer mentoring.
Core interventions: Family communication role-plays; structured family meetings; relapse-prevention plan for the mother; leadership/advocacy skills; “legacy of healing” ritual/project.
Objectives (3):
- Practice empowered communication and firm/compassionate boundaries;
- Build relapse-prevention for old emotional/relational patterns;
- Lead—at home, community, or peer settings.
Benchmark: By Week 52, ≥40% improvement in resilience/stress regulation (PSS, PCL-5) and demonstration of 1+ sustained lifestyle/relational change.
Primary modalities: Family Systems therapy, CBT relapse-prevention, strengths-based coaching, group therapy/peer mentoring.
